1. Background Checks:
If you wish to volunteer for any church ministry, group or event; you must complete this two- page Background Check Form to be eligible.  Please open and print the form and submit it completed with a copy of your State I.D. or Driver's License to the Main Parish Office. 
If you are not able to print this form at home, simply come by the Main Parish Office during business hours.  Thank you!
